Pilot Flight Report Debrief

Complete the following report after each official flight for credit.

Pilot Recommendations

If you observed a fellow pilot act bravely or heroically and believe that they should be considered for a special award, complete and submit the form below.

Describe in detail the what, when, where and whom of the event.  Be sure to include the names of other witnesses (friendly or enemy) who would be able to verify your report.  If there are no other witnesses, it is unlikely that your recommendation will result in an award.  However, do not let that prevent you from offering praise to another pilot.  It is possible that the investigating officer will recommend a letter of commendation, and there are always rare exceptions to the rule. 

Certification Reporting

Qualified Trainers Only.  Report certification flight outcomes after each component is completed.  Pilot Candidates should complete a Flight Debrief (above) so that staff may identify situations where the certifying pilot fails to complete this report.
